If you are unsure which form applies to your situation, please contact our admissions office before submitting documents. Many of the forms below are required only for students applying for an F-1 visa. Students who do not require an F-1 visa may only need to complete the Application for Admission.
Before You Submit Your Documents
Most F-1 visa applicants will need to submit the following items together:

Sponsor form (if someone else is financially supporting you)

Required for Most Applicants
1. Application for Admission (Domestic/Non-F-1 and International F-1 Students)
Complete the online application form to begin the admissions process.
Dependent Form (F-2)
Use this form if you are requesting documents for a spouse or child dependent.
Financial Support Forms
These forms are typically required for F-1 visa applicants who must demonstrate financial support.
2. Bank Account Certification Form
Submit this form as part of your financial documentation.

3. Form I-134 – Affidavit of Support
This form must be completed by a sponsor who is in the United States.

4. Certification of Financial Support
If your sponsor is not currently in the United States, they should complete this form and sign it in the presence of a notary.

Transfer Students
These forms apply to students transferring from another SEVP-certified school.
